Reasonable Accommodations Under the Americans with Disabilities Act (ADA)

The University of Illinois at Urbana-Champaign will comply with the requirements of the Americans with Disabilities Act and/or other applicable Federal or State law regarding employees or applicants who request an accommodation under those laws.

The University must analyze each request for accommodation on a case-by-case basis and make a good faith effort to reasonably accommodate a qualified employee or applicant with a disability. The Office of Equal Opportunity and Access provides forms and guidelines to assist units in complying with the requirements of this policy. Additionally, units should contact the applicable human resources office on campus for assistance. For civil service positions please contact the Staff Human Resources Office. Reasonable Accommodation Request Form (PDF format): This form is used: to record the accommodation request; to explore possible workplace accommodations; to provide a means of reviewing such requests; and, to record the University's response.

Essential and Marginal Job Function Analysis (PDF format): Under the guidelines of the Americans with Disabilities Act, units must, if applicable, analyze the essential job functions when a disability accommodation request is made by an employee or applicant for a position. Units are encouraged to complete the analysis prior to advertising vacant positions and use the information in the interviewing process. Assistance for determining when a unit must analyze the essential job functions is available from the Office of Equal Opportunity and Access.

Health Care Provider Release Form (PDF format): To be used by the employee or applicant requesting an accommodation to allow their health-care provider to advise the University about the requestors functional abilities and limitations in relation to their job functions.
